Occupational Injury and Illness
AFPM collects occupational injury and illness data from all regular member refining and petrochemical sites in the U.S. annually. The information collected is from the OSHA 300A Summary Log for both company employees and contractors (contractor aggregate). This data is used to compile the AFPM Survey of Occupational Injuries & Illnesses (I&I Survey). Sites that submit data may be eligible for AFPM Safety Awards.
AFPM will begin collecting the calendar year 2021 data in January 2022. Applications for the Distinguished Safety Award are due by January 31, 2022. Statistics for Occupational Injury and Illness will be due by February 22, 2022. To participate, each company must designate a database administrator who is responsible for managing the company's individual employee access and is responsible for reviewing all submissions.

Process Safety Event Metrics (754)
AFPM collects Tier 1 and Tier 2 Process Safety Event (PSE) data from all Regular Member Refining and Petrochemical sites for inclusion in an Annual Tier 1 and Tier 2 Process Safety Event Report. The report is available to the participating companies. Data collected is based on ANSI/API RP 754, Third Edition (August 2021) “Process Safety Performance Indicators for the Refining and Petrochemical Industries”. Companies may either submit data through the AFPM Safety Portal or submit an excel file of all events to Anna Scherer . Statistics for Process Safety Events will be due by March 4, 2022.
AFPM will begin collecting the calendar year21data in January 2022. AFPM will accept a company’s international sites in addition to their domestic sites. To participate, each company must designate a Database Administrator who is responsible for managing the company's individual employee access and is responsible for reviewing all submissions.
